Quality Home Care provides personal home care services to elderly and/or disabled consumers. We provide services in both Arizona and New Mexico and serve many Native American communities. The goal of Quality Home Care is to enable elderly and/or disabled clients to live in the comfort of their own homes and achieve the highest level of independence possible.
Services provided include but are not limited to the following:
•Mobility Assistance
Assist the consumer/client in transferring/moving from one place to another and assist with motion exercises for bed bound clients.
•Eating
Assist consumer/client with any aspect of feeding themselves.
•Meal Preparation
Prepare meals for the consumer/client 1 to 3 times per day. Preparation of snacks is also allowable 1 to 3 times per day.
•Assist with Self-Administered Medication
Remind the consumer/client to take their medication and open medicine packages for them.
•Bowel and Bladder Services
Assist with diaper changes and bladder programs, empty portable toilets, etc.
•Cognitive Assistance
Keep the consumer/client on task and increase safety primarily for people with traumatic brain injury, Alzheimer's disease, mental illness, etc.
•Household Services
Maintain a clean, safe environment for the consumer/client's personal
living area including assistance with their laundry, dishes and light household
cleaning.
•Hygiene/Grooming/Bathing
Assist the consumer/client with dressing, hair care, bathing, oral hygiene, nail care and shaving.
•Skin Care
Physician approved routine or preventive care.
•Minor Maintenance of Assistive Devices
Minor routine maintenance of walkers, canes, crutches, wheelchairs, oxygen tanks and other approved devices.
•Support Services (NM only)
Assistance with grocery shopping, doctor appointments, going to the
Post Office, to the barber/beauty shop, to quality of life events, paying
bills, etc.
•Other Services (AZ only)
Assistance with gathering wood, chopping wood and hauling water.
Anyone who has full Medicaid and needs help with two or more 'Activities of Daily Living' (including all the services listed above) and is 21 and over. We also offer private pay plans for anyone who does not qualify for NM Medicaid services but can pay on their own.

Steps to receiving personal home care in New Mexico
•Request for Service
For private pay customers call us and tell us what services you need. For Medicaid eligible consumers, call the Personal Care Options (PCO) information line to enroll in the program. For more information click here.
•QHC will Assist
Bring the packet to QHC before going to the doctor and we will assist you with completing the information requested.
•Medical Assessment
A physician must complete a Medical Assessment Form to determine the medical needs requiring assistance with Activities
of Daily Living. When
completed, bring it to QHC and we can fax it to the proper office.
•Independent Assessor
An Independent Assessor will be assigned to make a home visit to complete a personal care service plan to determine the medical necessity. This is sent to a Utilization Reviewer to be approved by the New Mexico Medicaid Program.
•Personal Home Care Service
Once the Utilization Reviewer approves the application for Personal Care, an attendant will be hired to fulfill the duties that are approved for service. A field coordinator will visit the consumer a minimum of once a month to ensure quality services are meeting the consumer's needs.
Anyone who is approved by Arizona Long Term Care Services (ALTCS), over the age of 64 or an individual who is disabled (e.g. blind, etc.), meets the financial requirements and needs nursing home level of care. Steps to receiving personal home care in Arizona
•Request for Service
For private pay customers call us and tell us what services you need. For ALTCS eligible customers who have
completed their application they will be interviewed by a nurse or social
worker to determine medical
eligibility. For additional questions, contact an Eligibility Specialist in the ALTCS local office.
•Long Term Care
If determined eligible, an ALTCS case manager will be assigned to you to make a home visit to determine needs requiring assistance in activities of daily living. The case manager will create a service plan listing the services you will need and how many hours per week an attendant will work for you.
•Personal Home Care Service
The case manager will then allow you to choose a home care agency to provide an attendant to fulfill the duties determined necessary by the case manager. A Field Coordinator will visit the client a minimum of once a month to ensure quality services are meeting the client's needs.

Do I need to prequalify to work here
Yes. Attendant applicants must agree to a nationwide background check, and/or cardiopulmonary resuscitation (CPR) & first aid training if not currently certified. As a federal Personal Care Options (PCO)
provider agency we must work closely with the state of New Mexico and
Arizona to ensure our PCO attendants meet all state and federal
requirements.
As an attendant/caregiver do I need to meet any other guidelines
Yes, we require that you must:
•Be eighteen years of age or older.
•Keep accurate time sheets and submit them weekly.
•Submit an I-9 form (proof of eligibility to work) to be kept on file.
•Adhere to the Medicaid Personal Care Service Plan.
•Include three references from non-family members (AZ only).
•Have a current Tuberculosis (TB) skin test or chest x-ray.
•Have current CPR, First Aid, Food Handlers and Home & Fire Safety Training Certificates (In NM, if you are hired you have 90 days to get these
certifications).
•Possess a valid state driver’s license and car insurance if transporting clients.
•Submit a 3-year MVD driving record (NM only).
