Quality Home Care provides home care services to elderly and/or disabled consumers.  We currently provide services in both Arizona and New Mexico and serve many Native American communities in both NM and AZ.  The goal of Quality Home Care is to enable elderly and/or disabled clients to live in the comfort of their own homes and achieve the highest level of independence possible. 

 

Quality Home Care also strives to provide home care services that promote a better quality of life for the individuals that QHC serves and their communities.  Our clients have the choice and flexibility as to how their home care is ultimately delivered.  We offer our clients the opportunity to have regular, friendly, caring, & respectful attendants available to help them at home with their activities of daily living.  QHC has a friendly, professional, and  respectful staff who are always ready and willing to help.  We regard our clients as friends and do what we can to ensure respectful and quality service.  Contact us if you or a loved one is in need of Quality Home Care.

 



Services provided include but are not limited too;


MOBILITY ASSISTANCE - Assist the consumer in transferring/moving from one place to another. May also assist with motion exercises for bed bound clients.

● EATING – Assist client with any aspect of eating.

● MEAL PREPARATION - Prepare meal(s) for the client 1 to 3 times per day. Preparation of snack(s) is also allowable 1 to 3 times a day.

● ASSIST WITH SELF-ADMINISTERED MEDICATION - Remind the client to take their medication and open medicine packages for them.

● BOWEL AND BLADDER SERVICES – Assist with Attends/briefs & bowel programs, empty portable toilets, etc.

● COGNITIVE ASSISTANCE - Reminding the client to help keep them on task and maintain safety.

● HOUSEHOLD SERVICES - Maintain a clean, safe environment for the client, may include assistance with the client's laundry, dishes and light household cleaning.

● HYGIENE/GROOMING/BATHING - Assist the client with dressing, hair care, bathing, oral hygiene, nail care, and shaving.

● SKIN CARE - Physician approved routine or preventive care.

● MINOR MAINTENANCE OF ASSISTIVE DEVICES - Minor routine maintenance of walkers, canes, crutches, wheelchairs, oxygen and other approved devices.

● SUPPORT SERVICES - Includes assistance with grocery shopping, doctor appointments, trips to the Post Office, going to a barber/beauty shop, going to quality of life events, paying bills, etc.

Who is eligible for services in New Mexico?

Eligibility for home care is provided to anyone who has full Medicaid (or is approved by ALTCS) and needs help with two or more "Activities of Daily Living" and is over the age of 21. "Activities of Daily Living" include all the services listed above. 


What if you do not have health insurance coverage for home care in New Mexico?

Apply for Medicaid health insurance coverage at any NM Medical Assistance Division.  We also offer private pay plans.


Steps to receiving personal home care services in New Mexico:

1st Step.

 REFERRAL - Referrals to a personal care provider may be made by the consumer, a family member, a friend, a health professional, or anyone aware of a need.

2nd Step.

 REQUEST FOR SERVICE - The consumer or their representative must call 1-866-364-3310 to request service. The consumer should receive a packet by mail in two weeks.

3rd Step. 

 QHC WILL ASSIST - Bring the packet to QHC before going to the doctor and we will give you instructions for completing the information requested.

4th Step.

 MEDICAL ASSESSMENT - A physician must complete a Medical Assessment Form to determine the medical needs requiring assistance in Daily Living. When completed bring it to QHC and we can help fax it to the proper office.

5th Step.

 INDEPENDENT ASSESSOR - An Independent Assessor will make a home visit and complete a personal care service plan which is sent to a Utilization Reviewer to be approved by the New Mexico Medicaid Program. The Reviewer will determine the medical necessity and if the Personal Care Service Plan meets the criteria for service.

Final Step.

 PERSONAL HOME CARE SERVICE - Once the Utilization Reviewer approves the application for Personal Care, an attendant will be hired to fulfill the duties set forth in the Personal Services Care Plan that are approved for service.  A field coordinator will visit the consumer a minimum of once a month to assure that quality services are being received and that the consumer's needs are being met.

Steps to receiving personal home care services in Arizona:


1st Step.

 REFERRAL - Referrals are made by CHR's, discharge planners, physicians, Community Health Nurses, and other concerned parties.

2nd Step.

 ALTCS APPLICATION - The Arizona Long Term Health Care System(ALTCS) program is for people 65 and over, blind, or disabled individuals who need ongoing services at a nursing facility level of care. However, program participants do not have to reside in a nursing home.

3rd Step.

 ELIGIBILITY - The income limit for this program is $2,200 per month, although persons with AHCCCS approved income - only trusts may have income in excess of this amount.

4th Step.

HOME VISITS - An ALTCS Eligibility Representative will make one visits to the home to determine if you are eligible for ALTCS by meeting health and financial requirement.

5th Step.

 LONG TERM CARE - If determine eligible, an   ALTCS case manager will be assigned to you and will make a home visit to determine needs requiring  assistance in activities of daily living. The Case Manager will create a service plan  that lists what services you will need and how many hours per week an attendant will work for you.

 
Final Step.

 HOME CARE SERVICE - The case Manager will then allow you to choose a home care agency to provide an attendant to fulfill the duties determined necessary by the Case Manager. A Quality Home Care Field Coordinator will visit the client a minimum of once a month to ensure that all services are received and that the clients needs are met.
