Who is eligible for services in New Mexico?
Eligibility for home care is provided to anyone who has full Medicaid (or is approved by ALTCS) and needs help with two or more "Activities of Daily Living" and is over the age of 21. "Activities of Daily Living" include all the services listed above.
What if you do not have health insurance coverage for home care in New Mexico?
Apply for Medicaid health insurance coverage at any NM Medical Assistance Division. We also offer private pay plans.
Steps to receiving personal home care services in New Mexico:
1st Step.
REFERRAL - Referrals to a personal care provider may be made by the consumer, a family member, a friend, a health professional, or anyone aware of a need.
2nd Step.
REQUEST FOR SERVICE - The consumer or their representative must call 1-866-364-3310 to request service. The consumer should receive a packet by mail in two weeks.
3rd Step.
QHC WILL ASSIST - Bring the packet to QHC before going to the doctor and we will give you instructions for completing the information requested.
4th Step.
MEDICAL ASSESSMENT - A physician must complete a Medical Assessment Form to determine the medical needs requiring assistance in Daily Living. When completed bring it to QHC and we can help fax it to the proper office.
5th Step.
INDEPENDENT ASSESSOR - An Independent Assessor will make a home visit and complete a personal care service plan which is sent to a Utilization Reviewer to be approved by the New Mexico Medicaid Program. The Reviewer will determine the medical necessity and if the Personal Care Service Plan meets the criteria for service.
Final Step.
PERSONAL HOME CARE SERVICE - Once the Utilization Reviewer approves the application for Personal Care, an attendant will be hired to fulfill the duties set forth in the Personal Services Care Plan that are approved for service. A field coordinator will visit the consumer a minimum of once a month to assure that quality services are being received and that the consumer's needs are being met.
