Introduction

Rides are provided for medical appointments, dialysis treatments, Medicaid-approved behavioral health services, hospital admissions and discharges. Our transportation vehicles are equipped with GPS Fleet Tracking and Satellite Communication equipment for the safety of our customers, drivers and the community.


Non-Emergency Services

We provide transportation for the following services:

•Dialysis treatment
•Medical appointments
•Physical therapy
•Hospital admissions/ discharges
•Child health visits
•Nursing home placements and other Medicaid-approved doctor visits
•Transportation for Navajo Nation Behavioral Health (RBHA) clients


How to Request a Ride

Call us at 1-888-878-RIDE(7433) at least 24 hours before appointment. Please have the following information ready when you call us:

•Patient’s name
•Date of birth
•Arizona AHCCCS ID #
•Social security #
•Mailing address
•Diagnosis (medical reason for transport)
•Appointment date, time and destination
•Directions to your pick-up location
•Name of person accompanying patient, if any
•Medical provider’s name
•Specific requests (wheelchair service, 1 on 1 driver assistance for the blind, etc.)


Eligibility

People who are enrolled in the following insurance programs may be eligible for FREE non-emergency transportation services:

•Arizona Medicaid (AHCCCS)
•Arizona Long Term Care Services (ALTCS)
•Navajo Nation Behavioral Health (RBHA)
•Developmental Disabilities (DD-DES)
•Disability Determination Services (DDS)

Health Insurance Coverage

Apply for Medicaid health insurance coverage at any Indian Health Service facility (contract health office) or at any food stamp office.


Employment Opportunities

Native Transport is always looking for safe and courteous drivers, age 26 and over with clean driving records. Download our application form by clicking here or pick one up at our Gallup branch office. Make sure to include a copy of your driver's license, Social Security Card and MVD driving record. Mail or deliver all documents to our Gallup branch office. We cannot process your application without all the required items.
